Currently we are accepting the following insurances:
*At this time we are only seeing patients of ages 18 and older.
**All patient’s must have Co-Pay ready at the time of appointment.
*** Patients are responsible to get a prior authorization from their primary care physician if their insurance requires it
Cancellations must be done a day or more prior to the appointment time.
Same Day Cancellations, Reschedules, or Missed appointments will be charged $50 fee.
