To schedule new appt: Call (702) 485-2100, Press one for scheduling; bring ID and insurance card to each appointment. Please make sure to arrive with your new patient packet completed. If you are unable to print paperwork; please arrive 45 minutes to an hour prior to appt to complete intake paperwork

• Make sure to arrive at least 45 minutes to an hour prior to appointment and all patients must have co-pay paid 24 hours prior to be seen.
• Patients are responsible to get a prior authorization from their primary care physician if their insurance requires it.
• Cancellations must be done 24 hours prior to appointment time.

Same Day Cancellations, Reschedules, or Missed appointments are subject to a cancellation fee of $50.

Medical Records:

With medical records we can upload them onto your patient portal, you would just have to fill out the “record release form”, or you can pick them up in office for .60 cents a page.
